Cancer Survivorship:  Creating a Personalized Care Plan

Vincent Cryns, MD, is medical director of the Breast Cancer Survivor Comprehensive Care, Empowerment and Education Program (SUCCEED), developed to address the unique needs of breast cancer survivors. A multidisciplinary team of physicians, nutritionists, exercise therapists, psychologists and others work with each survivor to provide education and comprehensive care, addressing cancer prevention and surveillance--as well as side effects and psychosocial concerns after treatment. Patients participating in the program, located in Prentice Women’s Hospital, within the Lynn Sage Comprehensive Breast Cancer Center, receive a “survivorship care plan” containing a detailed summary of diagnosis, treatment, and follow-up plans for the first year, including any future testing or interventions that may be needed. The event, hosted by the Lurie Cancer Center, Flourish Studios and Second Act, will begin with a networking reception, followed by Dr. Cryns’ lecture and time for questions.
